Default Small White Worms?

Meh. Even after a ridiculous amount of research, care, fasting, cleaning, ect., I still have these tiny white worm looking things. They don't seem to be harming my fish, but they are unsightly. They float around in the water and are buried in my substrate (Dirt capped with eco-complete). What are they and how do I get rid of them?

Planarian. Stop feeding too much. They will die if theres not any extra food to eat.

They are free swimming? Or stuck to glass?
