Yellowing leaves can be the sign of a variety of deficiencies. If it is only the new leaves that are yellowed, this is usually an indication that iron is lacking. In older leaves, it can indicate a nitrogen or a potassium deficiency. With a potassium deficiency, small pin holes often accompany the yellowing. Alternately, if phosphates are too low, this can limit the uptake of nitrogen even if there is plenty of ammonium or nitrates. As plant growth increases, you may need to increase the doses of some of the supplements.

Try a Flourish Tab (root tab). I always had great results using them on swords. They would grow like crazy. Also, do NOT plant them too deep. Make sure the crown is exposed.

What lighting are you using, how long are they on for per day? When did you buy the plants (how long have they been in your tank?). How long did it take the plants to start deteriorating? How much of each fertilizer are you adding per day or week? Can you post some close up good quality photos of the plant damage?

Question Re: amazon sword leaves keep yellowing

I agree with @Zapins, without answers to those questions, it is impossible to say. If they are newly planted, it may take a month or two for them to stop melting/yellowing.
