Can anybody please know what’s wrong with this plant?

I dose seachem: florish, trace, iron, potassium, phosphorus, nitrogen and excel.
Temperature: 28 c
Light: Advanced
I do 50% water change weekly

Could be a few things. Swords are heavy root feeder. Get root tabs, root fertilizers, and put it under the roots.
